A US passport replacement may be required for a variety of reasons. A name change, loss or theft or expiration are all possibilities. When a person changes his or her name, form DS-5504 must be submitted but only if the issuance date was within the previous year. Otherwise, form DS-82 must be submitted. It is possible to renew passports in the mail when this is the reason for a new one. A name change document like a court order or a marriage certificate must be in the renewal packet.

After this identification of origin expires, a person must acquire a new one. There are a few reasons a passport might expire. Adult passports expire every 10 years. Minor passports expire every five years or when a child reaches the age of 16 and must apply for a first-time adult passport. Adult replacement passports can be obtained through the mail if certain requirements are met. Form DS-82 must be used.

In order to get one by mail, it needs to be in a persons care and not have any damage. The old one needs to be sent in with the application. The item has to be under 15 years old. It has to have been acquired after a persons 16th birthday. Additionally, the name on this piece of identifying information should be up to date or any changes should be validated with attached documentation.

If a passport is stolen or lost getting a new one is a bit more complicated. The incident has to be reported before anything else can happen. There is a phone line dedicated to just such a purpose at the US state department. Form DS-64 is another way to report a loss or theft.

At the point that a report is made declaring an item missing, a new one can only be procured by filling out two forms. These forms include a DS-11, an application, and DS-64, a statement about the loss or theft. These forms have to be brought to an authorized passport location, they cannot be sent in the mail. If the incident occurs abroad, then a person has to go to the United States embassy or consulate in that country.

Each situation has a different fee associated with it. Without a rush, fees are lower. To have both a book and a card, it costs $140. A person who does not want a card will only be charged a fee of $110. There are cases in which a person already has their book wishes simply to add a card. This charge is only $30.

Getting a American passport replacement requires official documentation and several application forms. There are a variety of reasons like a name change or an expiration that one might need to obtain a new U S passports. In both those situations, it can be done by mail.
